    I have lost my debit card. I have applied new debit card in Kolhapur Axis Bank (Near Kamala College ) last one and haft month, till today not received debit card. Our office record shows debit card received on 14 th March 2012 but this date Pin number courier received on that day.
    This request is also noted in your office dated 28/03/2012 – No 6194109.
    So kindly request you send me Another Debit card.
